Release of Liability & Assumption of Risk
I, the undersigned owner, authorize Westside Walks and Jeremiah Acerra ("Provider") to provide dog walking, pet sitting, and related care services for my pet(s) listed above. I certify that all information provided is accurate and that my pet is in good health, current on vaccinations (including rabies), and free of any communicable disease.
I assume all risks associated with the care of my pet, including but not limited to injury, illness, escape, or death, except in cases of gross negligence by the Provider. I agree to release, indemnify, and hold harmless the Provider from any and all claims, damages, or expenses arising from services provided.
I authorize the Provider to seek emergency veterinary care at my expense if needed and to act in the best interest of my pet. I agree to reimburse any veterinary or related costs incurred during care.
I grant the Provider permission to enter my residence solely for the purpose of providing agreed services, and to use photos of my pet for promotional purposes unless I opt out in writing.
